JBMOnboard360 / HIPAA-ready intake forms
HIPAA compliant forms
Need HIPAA compliant forms? Intake that lands in the client record, under a BAA.
HIPAA compliant forms need two things: a vendor that signs a Business Associate Agreement, and a safe place for the answers to go. A form tool that emails submissions or sits apart from your CRM leaves PHI scattered. JBMOnboard360 builds intake forms with QR codes in its custom form builder, keeps the answers with the client's profile, and includes a BAA on every plan.
The basics
What makes an intake form HIPAA compliant?
A form is only as safe as the system behind it. If you're a covered entity, every vendor that collects, stores or sends your patients' answers needs a signed BAA (HHS). Then check four things:
- Encryption. Answers encrypted in transit when submitted and at rest once stored.
- Where answers go. Into a secure record, not an email inbox, a spreadsheet export or a shared drive.
- Who can see them. Only staff who need them for that client.
- Minimum necessary. Ask only for what you'll use. Every extra field is more PHI to protect.
Your side still matters: your risk analysis, privacy notice and staff training. No form builder makes a practice compliant on its own. Nothing here is legal advice; check your setup with your compliance advisor.
Is a HIPAA compliant form builder enough?
Often not. A standalone form builder with a BAA protects the form. But if staff then download the answers, email a PDF or retype them into a CRM without a BAA, the PHI leaves the safe zone. Digital intake forms work best when the form, the client record and the schedule are covered by the same BAA. When you compare form builders, check which plan includes the BAA and where submissions end up. Our BAA comparison of 18 vendors covers the CRM and scheduling side.
What you get
Digital patient intake forms, HIPAA-ready
A custom form builder
Build intake, consent acknowledgement and feedback forms with your own fields. Copy fields from our templates or write your own.
QR codes for check-in
Print a QR code at the front desk or in the treatment room. Clients fill in the form on their own phone, so no clipboards.
Answers in the client record
Submissions sit with the client's appointments, invoices and notes, not in an inbox or a separate tool.
A BAA on every plan
Included from $79 per user, or $399 per location with unlimited users on Business. Not an add-on.
US hosting, encrypted
AWS in the United States, encrypted in transit and at rest, with daily backups to a second US region.
Feedback after the visit
Send a feedback form after the appointment, and handle any complaint as a case in the help desk.
Field checklist
The fields a HIPAA-aware intake form needs
Use this as a starting list and remove anything you won't use. Clinical history beyond what your service requires belongs in your EHR, not your intake form.
Identity and contact
- Full legal name and preferred name
- Date of birth
- Phone and email
- Mailing address, only if you send mail
- Emergency contact name and phone
Communication consent
- Preferred contact method
- OK to leave a voicemail with appointment details? Yes / No
- OK to email appointment details? Yes / No
Visit-relevant health questions
- Reason for visit, in the client's words
- Allergies and current medications, if your service needs them
- Conditions that affect this service only
- Referring provider, if any
Acknowledgements
- Received the Notice of Privacy Practices (covered entities)
- Read the cancellation policy
- Typed full name and date
Leave out: Social Security numbers unless you truly need them, full medical history "just in case," and free-text boxes you never read. Want ready-made versions for salons, med spas and clinics? See our client intake form templates.
How it works
QR code intake, from front desk to client record
- Build the form in the form builder, using the field checklist above.
- Print its QR code for the front desk, or share the form link before the visit.
- The client scans and fills it in on their own phone.
- The answers are saved with that client's profile, next to the appointment.
- Staff with access review it before the visit. Nobody retypes anything.
Never collect intake by WhatsApp, text or email attachment. The JBMOnboard360 WhatsApp concierge is for general enquiries like prices, hours and booking a callback, never for health information.
When JBMOnboard360 is not the right fit
- You need e-signatures on legal consent forms. Ask us in the consultation what your consent workflow needs before relying on the form builder for it.
- You need intake to flow into an EHR. We don't claim EHR integrations. JBMOnboard360 is not an EHR or clinical documentation system; keep your clinical system for charting.
- You only need one form and nothing else. A standalone form builder with a BAA may be cheaper. JBMOnboard360 pays off when you also want the CRM, scheduling, invoicing and help desk.
Questions
HIPAA intake form questions
Are JBMOnboard360 intake forms HIPAA compliant?
They are HIPAA-ready. Forms are covered by the BAA included on every plan, and US data is hosted on AWS in the United States and encrypted in transit and at rest. Your policies, training and risk analysis complete it.
Do HIPAA compliant forms need a BAA?
If you are a covered entity or a business associate, yes. Any vendor that collects or stores your patients' answers needs a signed BAA before PHI goes in.
Can clients fill in intake forms on their phone?
Yes. Each form has a QR code you can print at the front desk, and a link you can share before the visit.
Can I send intake forms over WhatsApp?
No. Don't collect health information over WhatsApp. Share the secure form link or QR code instead, and use WhatsApp only for general enquiries.
What does it cost?
Forms come with every plan: from $79 per user per month billed annually, or $399 per location per month with unlimited users on Business. The BAA is included.
